Jen CPNP-PC will greet you. Please provide any forms you need completed at this time, or you can upload these while scheduling.
Forms that are completed at scheduling include your medical history and cardiac screening risk. Any additional questions will be asked prior to starting the exam.
The physical exam takes about 10-15 minutes, starting with basic inspection and vital signs including height, weight, and blood pressure.
A brief neurological screen includes inspection of the eyes, pupils, facial muscles, general strength of your extremities, ease of movement and balance. You may be asked to squat, or stand on one foot etc.
Your spine will be inspected during general stance, and while bending. This can indicate a curve in your spine, known as scoliosis, which generally does not limit activity but may require follow up at a later date.
FOR MALES: Some athletic programs request a testicular exam to screen for hernias. If your form indicates the need, Jen will discuss this with you and/or your parent.
If you have seen an eye physician within the year, the vision screen may be skipped.

FOR MALES:
Testicular check ONLY if indicated on your sports form and is approved by the athlete and parents.
Why is this needed? To screen for hernias that may pose a problems during intense activity.
Forms
If you have specific forms, you may upload these when you schedule your exam. Or simply bring them with you at the time of your exam.
If you do not have forms, Jen will provide one for you at the end of the exam, as long as you are cleared and deemed fit to participate.
